To Qualify For Services
We strive to provide the highest quality care with Christ’s love. We ask patients to provide necessary documentation including a valid photo ID and proof of income. Income eligibility is based on the poverty rates set by the US Department of Health and Human Services.
- A government issued photo ID.
- A household proof of income. This can be done by a Food Stamps Verification document, copies of current 2 months of paystubs, or an authorized Income letter (form provided by clinic).
